Blood Pressure or Blood Sugar Monitoring
If we have requested home monitoring blood pressure results or blood sugar results you can now email them direct to the nurse.
Please download either of the following documents - Home blood pressure monitoring form or Home blood sugar monitoring form.
Once completed please forward to oldccg.woodlandsmedicalpractice@nhs.net please state in the subject HBP and which nurse has requested the results.
